Planning Ahead for Your Health Care
Advanced Care Planning (ACP) helps you think about and share what kind of medical care you would want in the future. It’s about talking with your family and doctors so they know what matters most to you if you get really sick or hurt.
You can write down your choices in a legal paper called an Advance Directive. This helps your loved ones and your care team make the right decisions if you’re not able to speak for yourself.
With ACP, you can learn about different health conditions and treatments. This helps you make a care plan that fits your wishes and keeps your quality of life.
When your care choices are written down and shared, doctors and nurses can make sure you get the right care at the right time. ACP can also help you stay out of the hospital, avoid treatments you don’t want, and get the support you need, like hospice care, if the time comes.

- Talk to your Primary Care Provider, or any of your other care providers, and ask them for help referring
