Care Coordination
Care Coordinators help people who need extra support with their health and daily needs. They can:
- Help you make a plan to take care of your health.
- Work with you and your doctors to make sure you get the care you need.
- Teach you how to take care of your health at home.
- Help you make doctor or dentist appointments
- Get you help with food or housing
- Help you find a ride to the doctor, grocery store or pharmacy
- Connect you to local programs and resources
- And so much more!
You don’t have to figure things out on your own. We’ll work with you, your family, and your providers to make sure you get the care and support you need.

How do I get Care Coordination services?

- Self-Refer by sending an email to CaseManagement@umpquahealth.com
- Talk to your Primary Care Provider, or any of your other care providers, and ask them to refer you
Health Risk Assessment
A Health Risk Assessment (HRA) is a short survey that helps us learn more about your health and what support you might need. It asks questions about things like your health, home, food, and doctor visits.
When you finish your HRA, our care team can better help you stay healthy and connect you to services like rides, food, or health care. It’s quick and easy — and helps us take care of you!

Specialty Programs
Our Community Care Coordination & Navigation program helps you get the care and services you need to stay healthy. Our team works with you to make a plan, connect you to doctors, help with appointments, and find support for things like food, housing, and transportation.
We are here to answer questions, give support, and guide you every step of the way. Whether you need help with your health, finding services in your community, or understanding your care, our team is here for you.
Our Intermediate Case Management program is here to help members who need a little extra support with their health care. If you have health problems that need more attention, our care team can work with you to make a plan that fits your needs.
We can help you:
- Make doctor’s appointments
- Understand your health conditions
- Take your medicines the right way
- Find local help for food, housing, or transportation
- Talk with your doctors and care team
Our Complex Case Management program is for members who have serious or ongoing health problems and need extra help. If you have more than one health condition, have been in the hospital a lot, or need special care, our care team is here for you.
We will:
- Help you make a plan for your care
- Work with your doctors and care team
- Make sure you get the medicine and care you need
- Help with food, housing, and getting to appointments
- Support you and your family through your health journey
Our Hepatitis C Case Management program helps members who have Hepatitis C. This is a virus that can hurt your liver if not treated. Our care team is here to support you and help you feel better.
We can:
- Help you get the right treatment and medicine
- Answer your questions about Hepatitis C
- Work with your doctor and care team
- Help you with rides to appointments, food, and other needs
- Support you through every step of your treatment
