We are committed to improving the health of individuals and the community in Oregon through our coordinated care system. We are actively seeking new providers to join our network, which currently serves thousands of members in Douglas County through comprehensive physical, behavioral, oral, and dental care.

Umpqua Health Alliance (UHA) is one of Oregon’s 16 Coordinated Care Organizations (CCOs) and has been providing quality healthcare services to Oregon Health Plan (OHP) members since 2012. By joining our network, you can contribute to building a locally-driven health system that prioritizes community-based care and offers a broad range of resources to help you deliver the best possible care to your patients.

How to Join Our Network

Step 1: Submit an email request to our Contracting team, expressing interest in joining our provider network. Include your organization’s:

  • Practice name
  • Practice specialty type
  • Tax ID
  • NPI
  • Licensure type
  • Location
  • If you provide telehealth services

Step 2: Review our standard contracting process timeline.

Step 3: Submit all forms completely to help facilitate the timely processing of your contract request:

  • Provider Contracting Questionnaire

Effective Date will be determined upon credentialing approval date.

Step 4: Provider Relations will reach out for onboarding once contract date is assigned.

Who are we?

Umpqua Health Alliance (UHA) is one of 16 coordinated care organizations (CCO) contracted with the Oregon Health Authority (OHA) to coordinate the physical, behavioral, and dental health care services of Medicaid or Oregon Health Plan (OHP) members in a designated service area. UHA services the membership population within Douglas County, Oregon.

What is Umpqua Health Network (UHN)?

UHN is the entity that contracts and credentials the network of providers who see Umpqua Health members, including ancillary providers (i.e., durable medical equipment), hospitals, and facilities, such as home health, skilled nursing facilities, hospice, and laboratories. The UHN Agreement currently encompasses UHA (OHP), and Atrio Medicare Advantage Plans.

Who can become a contracted provider with UHN?

While anyone who provides services covered by UHN may apply to be a contracted provider, applying does not guarantee that a contract will be awarded.

How long does the approval/denial process take?

Once all necessary information/documentation is received by UHN, the process can take up to 2 months.

What goes in to deciding if I’m awarded a contract or not?

UHA takes a multi-faceted approach when determining network adequacy. Most importantly being patient need.

What is the first step?

  • PROVIDERS: To begin the process of joining our panel of providers, please email UHContracting@UmpquaHealth.com to request the Provider Request Questionnaire. Upon review, our Contracting team will follow up on next steps.
  • FACILITIES: Please email UHContracting@UmpquaHealth.com and request a Facility Request Questionnaire. Upon review, our Contracting team will follow up on next steps.

What may cause a delay?

Depending on the provider type and services offered, additional information may be needed. Untimely responses to requested information can extend review and/or decision timelines as negotiations need to be reviewed by Leadership.

Cultural Competence

Umpqua Health is dedicated to providing high-quality, patient-centered care that respects the diverse needs of our members. As part of this commitment, we have adopted the definition of cultural competence that appears in the Oregon Administrative Rules for Cultural Competence Continuing Education for Health Care Professionals (OAR 943-090-0010). This definition emphasizes the importance of health care providers’ ability to understand, appreciate, and effectively respond to the cultural and linguistic needs of their patients.

Cultural competence is a key component of our provider network and is integral to ensuring equitable care for all individuals in our community. We encourage all providers to enhance their cultural competence through ongoing education and training.
