What this Service Provides
The Advanced Illness Care program from Housecall Providers is for people living with serious or complex health conditions. The goal is to help you feel as well as possible by easing symptoms, lowering stress, and supporting your daily needs.
Care may be provided over the phone or through in-home visits. The team works with you, your primary care provider (PCP), and your specialists to help keep your care connected and focused.
Who is on the care team
The Advanced Illness Care team may include:
- Nurses
- Social workers
- Community health workers
- Spiritual counselor
How this program supports you
Who may qualify
This program supports people living with many kinds of serious or advanced illnesses, such as:
- Cancer
- Heart disease
- Liver or kidney disease
- Lung conditions such as COPD
- Diseases that affect the brain or nerves, such as Parkinson’s disease or ALS
- Immune conditions such as HIV/AIDS
- Other long-term or complex conditions
