PHI is any information about your health, like:
- Your health insurance or member ID number
- The name of your doctor
- What medicines you take
- When you went to the doctor
- Test results or diagnoses
How Do We Use Your Health Info?
We use your health information to:
- Help you get the care you need
- Pay for your healthcare services
- Work with your doctor or clinic
- Follow state and federal laws
We only use and share your information when we’re allowed to by law — and only with people who need it to help you.
We may share your information:
- With your doctor, clinic, or hospital to help with your care
- To pay for your care and make sure Medicaid benefits are used correctly
- With government programs that check healthcare quality or safety
- If there’s a safety emergency and someone is in danger
We will NOT share your health info:
- To sell or advertise to you
- Without your permission or another good reason permitted by law
Your Privacy Rights
As an Umpqua Health member, you have the right to:
- See your health records
- Ask for a copy of your records
- Fix mistakes in your records
- Ask us not to share your info in certain cases
- Choose someone to speak for you (like a parent or caregiver)
If you ever feel your privacy was not protected, you can file a complaint — and we promise you won’t get in trouble for speaking up.
