We want to make sure you and your baby are healthy and safe. Whether it’s your first baby or your fifth, we’re here for you every step of the way!
Maternity Case Management
Our Maternity Case Management program helps people who are pregnant and after their baby is born. Care coordinators can help you get to your doctor visits, learn about pregnancy and how to care for your baby, and find help with things like housing, food, and rides. We also help you get services like mental health support, help with breastfeeding, nurse visits at home, and care to keep you and your baby healthy.
New Day – Support for Pregnant Members
Our New Day program gives extra support to pregnant members who are facing or have faced mental health concerns or substance use. Care coordinators work closely with you to connect you to the care and services you need, like counseling, treatment options, prenatal care, and help after your baby is born. We also help with things like finding housing, getting rides to appointments, and making sure you and your baby stay safe and healthy.
New Beginnings – Support for Children Ages 0-5
New Beginning is a program that helps children from birth to age 5 and their families get the care and support they need. Our team works with parents and caregivers to make sure kids are healthy, growing, and reaching important milestones. We help with doctor visits, finding resources like food and housing, and answering questions about your child’s health and development. We’re here to support your child’s bright future!
Family-Centered Care
Family-Centered Care means we work closely with parents, guardians, and children to support your health. We listen to your thoughts, answer your questions, and respect your choices. We know that parents and guardians are the most important people in a child’s life, so we include you in making health care decisions. Our goal is to work as a team with your family to help your child grow up healthy and strong.
Parental Support
Being a parent or guardian can be hard sometimes, and it’s okay to ask for help. We can connect you to programs that have tips on parenting, help with finding childcare, and local programs for food, housing, and more. You’re not alone—we’re here to support you and your family!
