2025 Funding Opportunity
UHA is excited to announce that, in partnership with OHA, we will be awarding Community Capacity Building Funds (CCBF) again in 2025. These funds are designed to create robust, equitable networks of HRSN Service Providers across the state and build the necessary capabilities and capacity of community partners.
Funding Priorities
For 2025, UHA has $2,024,611 available to award, and we have established several funding priorities (subject to change):
- Focus on local providers in Douglas County or neighboring areas.
- Continue building network capacity for HRSN Housing services, with a focus on partners who can accept referrals for all HRSN Covered Populations and are able to provide ongoing Tenancy Case Management services.
- Build network capacity for HRSN Nutrition Services, in particular the Fruit & Vegetable benefit which is expected to launch in late 2025 or early 2026.
- Expand our Outreach & Engagement efforts, partnering with one or more Providers who can play a significant, consistent role in gathering the necessary information from Members to complete HRSN eligibility screenings.
Applications & Requirements
Applications will be evaluated based on adherence to CCBF guidelines and alignment with UHA’s funding strategies and HRSN program goals.
The application window for 2025 funding has closed. Organizations awarded funding by UHA will be required to execute a Master Funding Agreement (MFA) and our HRSN Service Provider Contract. This will include enrollment with the state as an HRSN Service Provider, and adherence to various contract requirements including but not limited to: referral and service delivery turnaround times, reporting minimums (both related to CCBF and the services you are providing), maintaining necessary insurance, completing provider-related training, and utilizing the funds in accordance with the MFA and your approved application.
