Community Health Improvement Plan
Our Community Advisory Council (CAC) provides annual funding for Community Health Improvement Plan (CHIP) projects aimed at creating a healthier Douglas County.
SHARE is a funding opportunity offered by Umpqua Health to community partners that can be used to address social determinants of health, which are conditions in which people are born, live, work, and play that affect a wide range of health outcomes and risks.
System of Care
We are a collaborative network of community-based programs that come together to address community needs, identify barriers faced by children and their families, and uncover unmet needs within the region.
Comprehensive Behavioral Health Plan
In 2021, the Oregon Health Authority required Coordinated Care Organizations (CCOs) to develop regional Comprehensive Behavioral Health Plans (CBHP). Umpqua Health engaged our community collaboratively in the development of our CBHP.
Community Capacity Building Funds
Umpqua Health is excited to announce that, in partnership with OHA, we will be awarding Community Capacity Building Funds (CCBF) again in 2025. These funds are designed to create robust, equitable networks of HRSN Service Providers across the state and build the necessary capabilities and capacity of community partners.
General Requests
While we offer various grant and funding opportunities throughout the year, we understand that not all needs align with a specific grant cycle. To ensure we can support a wide range of community efforts, we welcome general requests through our Project Application: Partnering for Success, as well as through our Community-Level Interventions: Request for Partnership.
