Umpqua Health is investing in the community through a Community Benefit Initiative (CBI) by offering Iris’ Healthcare’s Empower Advance Care Planning (ACP) platform to all Douglas County residents over the age of 18 with the goal of improving population health and health care quality. This benefits the entire Douglas County community at large, not just Umpqua Health Alliance members, and is intended to improve care delivery and overall member and community health and well-being. This digital platform will provide advance care planning access to those most in need such as low-income families, uninsured individuals and those without access to physicians or care teams.

By providing education about disease progression and the risks and benefits of different treatment options, individuals can create a care plan that truly enables them to achieve their quality-of-life goals.  Likewise, by documenting and distributing these care decisions, ACP ensures that individuals’ care preferences are understood and followed throughout their healthcare journey, creating a higher quality care experience overall.

Empower is a secure, self-guided online platform that simplifies the advance care planning process. Empower provides supporting information to help users make their future healthcare decisions, auto-generate state-specific advance directives and to sign, store and share their documentation with family members and care teams in the event of a medical emergency should they not be able to speak for themselves.

What is Advance Care Planning?

Advance care planning (ACP) is defined as a process that supports individuals in understanding and sharing their values, goals and preferences for future medical care. These care decisions are then formally documented in an Advance Directive, a forward-thinking legal document that empowers individuals, their loved ones and their care teams to make the right healthcare decisions moving forward. ACP facilitates physicians providing patients with the right care, at the right time, in the appropriate setting. ACP is associated with reduced hospitalization, receipt of less intensive treatments, increased utilization of hospice services.

What is Empower? 

Iris Empower is a self-guided online application that simplifies the advance care planning process with guiding prompts, supporting information and pre-populated documents, creating easy access to advance care planning. With Empower, individuals can revisit and update their care goals over time.

Who is it offered to? 

Umpqua Health has partnered with Iris Healthcare to offer Empower to all residents of Douglas County over the age of 18.

Why is it being offered? 

The ideal time to have healthcare planning discussions is when you are feeling well – just like financial planning, which works best when done years in advance of retirement. The earlier you plan, ideally before you reach a health crisis, the more you will benefit.

How does ACP allow me to get the care I want?

ACP is all about you having a voice in your future care. Through the conversation you will get to say what kind of care you want and then create legal documents that ensure your doctors honor your wishes.

How does ACP improve my quality of care?

ACP protects you from receiving unwanted or unnecessary medical interventions and lets you get just the kind of care you want. There is strong evidence that this kind of planning improves the experience of care for both the patient and their caregivers / family.

What if I change my mind or my situation changes? Can I update these forms?

Absolutely! You remain in control. You can make changes to these as often as you like. Your plan of care remains flexible and your wishes should be communicated to healthcare providers every time you are admitted to a healthcare facility and at least annually with your primary care provider.

Will healthcare providers/doctors, hospitals, and nursing homes respect my choices, once I complete these documents?

Healthcare providers have a duty to respect your wishes when providing your care, and there is good evidence that communication of your thoughts to family and healthcare providers is essential to ensuring your care is carried out based on your wishes. Without this personal information, decisions can easily result in providers focusing on standardized medical protocols instead of your particular needs.
