UMPQUA HEALTH MEMBERS

Starting in 2024, certain Oregon Health Plan (OHP) members may be eligible for extra non-medical services and supports. These services help address barriers to health. Below you can learn about each benefit. You can also learn about who is eligible, and how to apply.

Climate Supports

Available Services

  • Air conditioners
  • Heaters
  • Air-filtration devices
  • Mini refrigerators (for storing medications)
  • Portable power supply units (for powering medical devices during emergencies)
  • Installation support (unboxing and plugging in the device)

Who is eligible for Climate Services?

For all climate devices, you must be in at least one of these groups:

  • An adult or youth discharged within the past year from mental health or substance use disorder Institution of Mental Disease.
  • An adult or youth released within the past year from incarceration.
  • Involved in the child welfare system at some point in your life.
  • Within the next three (3) months or past nine (9) months, you are transitioning from Medicaid-only to dual coverage (receiving both Medicaid and Medicare).
  • Houseless or at risk of becoming houseless.
  • A young adult, aged 19-26, with Special Healthcare Needs (eligible starting in 2025).

You must also:

  • Be able to use the device safely.
  • Have a health condition that makes climate events challenging or dangerous.
  • Live in a place with electricity.

Important Housing Updates

Are you planning to apply for HRSN housing benefits? Read this first.

  • Due to high demand, decision times for housing requests are longer than usual. You can expect a minimum of 30-90 days before a decision is made. To help this process go more quickly, please complete the entire request form and send us all required documents. Delays in collecting this information can further delay your request.
  • Not all OHP members will qualify for HRSN housing benefits. Due to federal requirements, to qualify for HRSN housing benefits you must have specific health, living, and financial conditions.

Who is eligible for Housing Services?

For support with rent and utilities, you must:

  • Be currently housed with a written agreement or lease signed by you and your landlord.
  • Be at risk of becoming houseless and meet certain income criteria.

For services such as wheelchair ramps, grab bars and pest control, you must be in at least one of these groups:

  • An adult or youth discharged within the past year from mental health or substance use disorder Institution of Mental Disease.
  • An adult or youth released within the past year from incarceration.
  • Involved in the child welfare system at some point in your life.
  • Within the next three (3) months or past nine (9) months, you are transitioning from Medicaid-only to dual coverage (receiving both Medicaid and Medicare).
  • At risk of becoming houseless.
  • A young adult, aged 19-26, with Special Healthcare Needs (eligible starting in 2025).

For all Housing services, you must also:

  • Have a health condition that would be improved by the service.

Can HRSN help members facing eviction?

HRSN housing benefits aren’t meant for emergencies. It takes time to approve and receive services. If you have been given a court date because you’re behind on rent, please contact our partner UCAN instead of submitting an HRSN Housing Request to us.

Nutrition Supports

Available Services

  • Nutrition education
  • Medically tailored meals
  • Fruits and vegetables (available in 2026)
  • Pantry stocking (available in 2026)

Who is eligible for Nutrition Services?

For medically tailored meals and nutrition education, you must be in at least one of these groups:

  • An adult or youth discharged within the past year from mental health or substance use disorder Institution of Mental Disease.
  • An adult or youth released within the past year from incarceration.
  • Involved in the child welfare system at some point in your life.
  • Within the next three (3) months or past nine (9) months, you are transitioning from Medicaid-only to dual coverage (receiving both Medicaid and Medicare).
  • At risk of becoming houseless.
  • A young adult, aged 19-26, with Special Healthcare Needs.

You must also:

  • Meet with a Registered Dietician to develop a nutrition care plan. Only required for medically tailored meals.
  • Have a health condition that would be improved by the service.
  • Must be experiencing low food security.

How to Apply

If you are an Umpqua Health member, we can help determine if you are eligible. The first step is to complete an HRSN Service Request Form. You can complete the form yourself. Or, you can work with or someone you trust to complete the form. This could be your doctor, a family member, or a friend.

Application Review

Your request may take up to 14 days or more to be reviewed. Delays in receiving information will delay up to 28 days. Due to high demand, review times for housing requests are longer than usual. You can expect a minimum of 30-90 days before a decision is made.

  • If you qualify, we will refer you to an HRSN Service Provider. They will help you get what you need. It may take up to 30 days after approval to receive services. You’ll receive these services at no cost. You can opt out of receiving these services at any time.
  • If you don’t qualify, we will send you a letter by mail. If you think we’ve made a mistake, you can ask for an appeal.
