What is a Primary Care Provider (PCP)?
A primary care provider (PCP) is your main doctor and should be contacted first for medical issues. They are health professionals who take care of your general health. They can also refer you to other specialists, making sure you get the right care.
Who is Your PCP?
When you join Umpqua Health, we will mail you an ID card with your Member Packet. Your ID card will provide the name of your assigned clinic. You can contact your clinic to meet with your PCP.

Change Your PCP
We understand the importance of having a primary doctor who meets your needs. If you feel that a different PCP would be a better fit, you can make that change. To change your PCP, please follow these steps:
Step 1 – Use our Provider Directory to browse available PCPs in our network.
Step 2 – Once you’ve found a new PCP, contact our Customer Care team and ask them to reassign you.
