What geographic area does Umpqua Health serve?

We serve most of Douglas County, Oregon, excluding some areas in Reedsport, Gardiner, Winchester Bay, and Scottsburg.

What are Umpqua Health’s hours of operation?

Customer Care is available Monday through Friday, 8:00 a.m. to 5:00 p.m.

Who can join Umpqua Health Alliance?

Anyone enrolled in the Oregon Health Plan (OHP) and living in UHA’s Douglas County service area may join.

How much does it cost to be a member?

Most members pay no monthly premium. OHP is a free health plan for those who qualify.

Call Customer Care. PCP changes are effective the same day you call.

Do I need a referral to see a specialist?

Some services require referrals, but others like women’s health, family planning, and behavioral health services may not. Learn more here.

How do I file a grievance or complaint?

What if I need interpreter services or materials in a different format?

We offer free interpreter services and materials in other languages, large print, Braille, and audio.

What do I do if I’m calling on behalf of a member?

You must have authorization on file or provide legal documentation like a Power of Attorney or signed Release of Information.

What benefits does Umpqua Health provide?
