Your Health Information: How We Keep It Safe

At Umpqua Health, we care about your health — and your privacy. That means we work hard to protect your personal health information, also called PHI.

PHI is any information about your health, like:

  • Your health insurance or member ID number
  • The name of your doctor
  • What medicines you take
  • When you went to the doctor
  • Test results or diagnoses

How Do We Use Your Health Info?

We use your health information to:

  • Help you get the care you need
  • Pay for your healthcare services
  • Work with your doctor or clinic
  • Follow state and federal laws

We only use and share your information when we’re allowed to by law — and only with people who need it to help you.

When Do We Share Your Health Info?

We may share your information:

  • With your doctor, clinic, or hospital to help with your care
  • To pay for your care and make sure Medicaid benefits are used correctly
  • With government programs that check healthcare quality or safety
  • If there’s a safety emergency and someone is in danger

We will NOT share your health info:

  • To sell or advertise to you
  • Without your permission or another good reason permitted by law

Your Privacy Rights

As an Umpqua Health member, you have the right to:

  • See your health records
  • Ask for a copy of your records
  • Fix mistakes in your records
  • Ask us not to share your info in certain cases
  • Choose someone to speak for you (like a parent or caregiver)

If you ever feel your privacy was not protected, you can file a complaint — and we promise you won’t get in trouble for speaking up.
