HRSN Service Providers

Health-Related Social Needs (HRSN) is a new benefit program that began rolling out in 2024 for eligible Oregon Health Plan (OHP) members. These services help address barriers to health that may affect well-being.

What is an HRSN Service Provider?

HRSN Service Providers are organizations that deliver one more types of HRSN services to members. Learn more about the services available through this program, provider qualifications, and how to become part of our network below.

HRSN Services

Outreach & Engagement

  • Engaging OHP members who may be eligible for HRSN services
  • Verifying the member’s CCO or open card enrollment
  • Sending an HRSN request to the member’s CCO, or OHA for open card members
  • Connecting members to medical, peer, social, educational or legal services

Climate

  • Air conditioners
  • Heaters
  • Air-filtration devices
  • Mini refrigerators (for storing medications)
  • Portable power supply units (for powering medical devices during emergencies)
  • Installation support (unboxing and plugging in the device)

Housing

  • Rent and utility assistance
  • Tenancy support services
  • Home changes for health and safety

Nutrition

  • Nutrition education
  • Medically tailored meals
  • Fruits and vegetables (coming in 2026)
  • Pantry stocking (coming in 2026)

Provider Qualifications 

In general, for all HRSN benefits, HRSN Service Providers must: 

  • Have knowledge of principles, methods and procedures of the service, or comparable services meant to support individuals in meeting related needs. 
  • Have the ability to appropriately deliver the service within a reasonable timeframe.  
  • Have the ability to participate in Closed Loop Referrals. 
  • Be accessible to Members, including having the operating hours and staff necessary to meet the Members’ needs. 
  • Demonstrate ability or experience to effectively serve at least one of OHA’s Priority Populations. 
  • Demonstrate provision of culturally and linguistically appropriate, responsive and trauma-informed services. 
  • Provide documentation that demonstrates a history of responsible financial administration.

How to Become a Provider

If your organization is interested in becoming an HRSN Service Provider for Umpqua Health, you can request an application form by emailing UHContracting@umpquahealth.com.

Enroll with OHA

You will need to enroll in OHA’s MMIS Provider Portal as an HRSN Service Provider. Follow OHA’s MMIS Provider Portal Provider Enrollment Guide and apply online. For help, find information and forms on the Provider Enrollment webpage or call the Provider Enrollment Team at 800-336-6016, option 6 to ask questions.

If approved, OHA will send a “welcome” letter. The letter will list your provider ID. It also has links to helpful information. If your application is missing any information, OHA will contact you and tell you what is needed.

Certification & Contracting

After you enroll in MMIS and submit your completed application to us, we will conduct a Certification process to ensure you meet all HRSN Service Provider qualifications. Note that if you are a registered dietician or a traditional health worker, you will also need to complete our Provider Credentialing process.

If your application is approved, our Contracting Department will work with you to establish an HRSN Service Provider Agreement.

Onboarding

Once a contract is in place, our Provider Relations team will engage you in our HRSN Service Provider onboarding process. On a high level, this includes:

  • General HRSN program training
  • Service-specific training based on your contract (Climate, Housing, etc.)
  • Privacy & Security training
  • Fraud, Waste & Abuse training
  • Getting access to necessary systems & resources
  • Completing a pre-launch readiness assessment
  • Collaboratively completing your first Closed Loop Referrals, service deliveries and invoices

HRSN Service Provider FAQ

Do I have to provide all HRSN benefits in order to be an HRSN Service Provider?

No, HRSN Service Providers can be focused on one or more HRSN benefits.

What is a Closed-Loop Referral?

“Closed Loop Referral” means the process of exchanging information between and among CCO, FFS Program, OHA, a Member, HRSN Service Providers, and other similar organizations, to make referrals and communicate about the status of referrals for a Member. HRSN Service Providers must be able to participate in closed-loop referrals.

UHA supports multiple closed-loop referral pathways, including:

  • Email
  • Fax

How are HRSN Service Providers reimbursed?

OHA has published an HRSN Fee Schedule that outlines the maximum payment amounts for each service. When you receive a Closed Loop Referral from us, we will indicate the approved budget for the service. You can find the current fee schedule under “Billing for HRSN Services” on OHA’s webpage.

You can submit an invoice to UHA within 120 calendar days of delivering an HRSN service. You will receive payment within 30 days of submitting your invoice. If your invoice is rejected for any reason, you can correct and resubmit within 120 days.
