What Housing Support Can I Get?
- Help paying rent or utility bills
- Help working with your landlord to stay housed
- Changes to your home to help keep you safe and healthy
Who Can Get Housing Support?
You may be able to get help paying rent or utility bills if you:
- Have a written lease or rental agreement signed by you and your landlord
- Are at risk of losing your housing
- Meet the income requirements
You may be able to get home changes, such as wheelchair ramps, grab bars or pest control, if you:
- Left a mental health or substance use treatment program in the past year
- Left jail, prison or another correctional setting in the past year
- Have ever been in foster care or another child welfare program
- Are moving from Medicaid to both Medicaid and Medicare. This can be within the next three months or during the past nine months.
- Are at risk of losing your housing
- Are 19 to 26 years old and have special health care needs
For any housing service, you must also:
- Have a health need the service may help.
Before you apply, you may need a copy of your lease and proof of income.
Most housing requests are reviewed within 30 to 90 days after we receive your application.
You can fill out the form yourself or ask someone you trust to help you. This could be your doctor, a family member, a friend or a community health worker.
