What is H.R. 1?

Also known as the “One Big Beautiful Bill Act”, H.R. 1 is a new federal law that will require changes to the Oregon Health Plan (OHP). These changes will start in late 2026 through 2028. The changes that apply to you will vary, but could include:

  • More frequent renewals
  • Shorter time period for help with recent medical bills
  • Work or activity requirements
  • A small copay may apply for some healthcare services
  • Changes to OHP for many immigrants

The federal government has not released details about all of these rules yet. You can find more information on some of these changes below. You can also stay up to date with the latest information on OHP’s federal changes webpage.

More Frequent Renewals

New federal rules will soon require more frequent renewals for OHP.

  • Most adults from age 19 through 64 will need to renew every six months.
  • Everyone else will renew once a year. This will include children and adults age 65 and over. 

Six-month renewals will not apply if: 

  • You are American Indian/Alaska Native.
  • You have Medicare or have OHP based on a disability.
  • You are on OHP Bridge. OHP Bridge renews once a year. 

The federal government has not shared all the details about the new rules, such as when more frequent renewals need to start in Oregon. Learn more about these changes by reading OHA’s fact sheet.

Work or Activity Requirements

Starting in 2027, when someone applies for or renews their OHP coverage, they will be reviewed for: 

  • An exemption, which means you have a reason you do not need to meet work or activity rules. Learn more about exemptions by reading OHA’s Exemptions Explainer. If you have both OHP and SNAP, read this document to learn more about how these changes may affect you.
  • Meeting one or more of these rules:
    • Income over $580 a month; or
    • Enrolled in higher education at least half-time; or
    • A combination of work, volunteering, education, or training programs that add up to at least 80 hours a month.

Changes to OHP For Many Immigrants

Oregon offers full OHP benefits to people who meet income and other rules regardless of their immigration status. People who do not meet the federal immigration status requirements for Medicaid receive a category of OHP called Healthier Oregon. Changes to the federal immigration status rules will mean about 7,000 adults who have certain immigration statuses will move to Healthier Oregon from other categories of OHP on October 1, 2026. Unlike in other states, people will not lose OHP due to this change.

Then, in January 2027, federal changes mean all Healthier Oregon OHP members will move to OHP Open Card. If you are a Healthier Oregon member, OHA will send you a letter about this change with more information in October 2026.

How to Prepare

Update Your Info

The best thing you can do right now is to keep your contact information up to date so OHP can reach you. You must tell OHP if your mailing address, email, phone number, or other information changes. OHP will send you notices when it is time to renew.

You can report changes online, in person, by phone or by mail. Learn more about when and how to report changes to OHP on their website. You can also ask an OHP-certified community partner for help.

Gather Your Documents

When it comes time to renew (OHA will send you a letter), you may need to submit evidence to show you are still eligible. It can help to have this information ready to go in advance! Learn more here.
