HRSN Home Changes for Health

Some Oregon Health Plan (OHP) members may be able to get items that help keep them safe and healthy during extreme weather or emergencies.

What Home Changes for Health Can I Get?

  • Air conditioners
  • Heaters
  • Air filters
  • Small refrigerators to store medicine
  • Portable power supplies for medical devices during an emergency
  • Help setting up the item, such as taking it out of the box and plugging it in

Who Can Get Home Changes for Health?

You may qualify if you :

  • Left a mental health or substance use treatment program in the past year
  • Left jail, prison or another correctional setting in the past year
  • Have ever been in foster care or another child welfare program
  • Are moving from Medicaid to both Medicaid and Medicare. This can be within the next three months or during the past nine months.
  • Are at risk of losing your housing or do not have a safe place to live
  • Are 19 to 26 years old and have special health care needs

You must also:

  • Be able to use the item safely
  • Have a health condition that makes very hot, cold, smoky or unsafe weather dangerous for you
  • Live in a home with electricity

Before you apply, your doctor or care team may need to confirm your health condition.
Most requests are reviewed within 14 to 28 days after we receive your application.
You can fill out the form yourself or ask someone you trust to help you. This could be your doctor, a family member, a friend or a community health worker.
